Under the leadership of the Associate Director, Nursing, the primary responsibilities for this position consist of providing total, comprehensive, continuous, and patient centered care which includes: assessing, identifying, documenting, planning, and administering nursing treatment for actual or potential health problems to students and employees who walk-in seeking care. Additionally, this position will facilitate the review process of pre-health screening forms of all in-coming students and case management of students with complex, complicated health issues. In the Student-Centered Medical Home (SCMH), the Assistant Nurse Manager will administer direct care to patients by performing those aspects of triage and nursing care requiring professional judgment and skills. The Assistant Nurse Manager will administer processes for pre-health screening reviews, manage complex health issues for in-coming students. Inherent in this role is independence and autonomy of practice within the framework of established guidelines.
Delivery of care is inclusive of pre-health screening review for incoming students, and coordination care management urgent care nursing, pregnancy screening, women's and men's health counseling, travel medicine, administration of vaccines, medications, allergy shots, and ear irrigation, crisis intervention, assessing acute and chronic medical problems; coordinate care for patients with PCP and practice teams; point of care testing; follow-up of laboratory results, referrals to the emergency department and consultants including facilitating receipt of reports to and from outside providers as needed. Refers patients for further clinical evaluation and follow-up, either immediate care or future care appointment using electronic health record.
Day-to-day collaboration with designated clinician, as well as managers, directors, practice team clinicians, health service assistants, and medical assistants to manage unscheduled appointments, telephone triage, educational needs, and delivery of nursing care.
Function as a member of Medical Services clinical team supporting patient scheduling and triage processes. Under the direction of the designated clinician of the day and working in unison with the clinical practice team leaders, manage unscheduled appointments, telephone calls and the flow and care of patients to be triaged. Applies evidenced-based practice when administering nursing care and performing therapeutic measures.
Evaluates and assess the severity of patient's health concerns utilizing established triage protocols and guidelines. Triage patients' complaints using critical assessment skills to include crisis intervention, acute and chronic medical problems, and women's health related concerns assigning acuity level, and prioritizing the needs of the patients seeking care timely and appropriately.
Appropriately obtain input and direction from providers when needed to determine best clinical course of action and recommendations for specific patient health care needs.
Accurately document symptoms/complaints, nursing assessment, triage plan and follow up in the patient's electronic medical record.
Performs telephone triage.
Participate in the development, review and revision of Nursing Triage protocols and practice guidelines and the development of standards, patient care policies and clinical procedures and audit peer practice to assure overall quality of patient care.
Effectively conducts a focused health history and performs a physical examination as indicated by patient's reason for visit.
Responds to emergency situations; ensure continuous reassessment and management of patients who remain in the waiting room.
Performs routine and specialized nursing care in areas of health screening and prevention, health assessment, treatment, and health education.
Coordinates care for patients with women's health and emotional problems, lab work with follow-up on results
Administers appropriate medications and treatments as prescribed by onsite providers.
Provides health education for acute and chronic medical conditions in a self-care model as well as wellness prevention model to patients and the Columbia University community.
Orders and interprets point of care diagnostic tests.
Works collaboratively with clinicians in planning for patient care and makes referrals to other disciplines as indicated.
Evaluates and documents patients' responses to the plan of care in the electronic health record system.
Works with Nurse Manager and Practice Team Nurses on follow-up of referred patients to the emergency department, consultants or discharged with self-care instructions.
Facilitates and participates in the review of pre-health screening forms of incoming students.
Works in a collegial/team model of nursing with nursing staff, clinicians, medical assistants, laboratory staff, Health Services Assistants, nutritionists, and in-house consultants (acupuncturist and athletic trainers) in assigned designated Practice Groups.
Collaborates with practice team leader during morning, evening huddles and team meetings.
Serve as a clinical resource for clinicians, other nurses, medical assistants, and non-medically trained staff.
Facilitates and participates in the coordination and continuity of care of patients with chronic, complex, or complicated health issues.
May be required to assist in interviewing and mentoring of newly hire Nurses.
Will be trained in travel medicine in-order-to provide travel assessment to students which includes advice on preventing and managing illness and injury while traveling as well as appropriate vaccines and medications.
Participates in all QA activities (self-evaluation, peer review and QI activities) interdisciplinary and inter-departmental committees as appropriate. Develops and evaluates own goals for professional growth.
Performs all duties and services in full compliance with Columbia University as well as Columbia Health policies and procedures.
Performs other related duties as assigned.
Requires Bachelor of Science in Nursing (BSN) and current New York State RN licensure.
Knowledge and experience with a wide range of illness and injury patterns in-order to conduct rapid and accurate triage based on clinical urgency of patients presenting problems.
Candidates must have superb direct patient care and critical thinking skills coupled with superior triage/assessment skills.
Proven ability to communicate in a professional and respectful manner with patients, co-workers and providers.
Strong organizational, prioritization skills, emotional stability, attention to detail, ability to work in a team as well as independently and being both decisive and sympathetic are crucial to this role.
Specific experience desired in triage, emergency health care, ambulatory care, occupational health, medical/surgical, college health, community health, patient teaching, health counseling, and health education.
Certified Emergency Nurse certificate, successful completion of a comprehensive triage education program, training and professional development in triage and related emergency nurse/emergency care activities strongly preferred.
Must be computer literate, familiarity with EMR and patient scheduling system.
Completion of current CPR Course and Infection Disease/Control Certificate.
