The primary responsibility of this position is to provide outreach for the Minority AIDS Initiative program. The candidate should have the experience and/or knowledge level to function independently in that role. Responsibilities will include providing of linkage and retention services to HIV-positive clients; HIV rapid testing, counseling and appropriate referrals to HIV care, partner notification and/or pre-exposure prophylaxis (PrEP) services.
A HIV Outreach Worker is responsible to link HIV identified clients primarily from the Prison system to HIV services and should have a thorough knowledge of psychosocial services and HIV service linkages in the Baltimore EMA and Ryan White HIV care network and why those services are integral to achieve medication adherence for viral suppression, optimal health outcomes and reduction of community transmission of HIV. Other duties may include community education; development and facilitation of support groups; participation in Johns Hopkins community advisory boards; and knowledge of research protocols specific to relevant populations.
The HIV outreach Worker will act as a representative of the Johns Hopkins Women’s HIV Program at a local and national level and may participate in public speaking events or trainings.
Specific Duties & Responsibilities
Travel throughout Maryland to complete intake on HIV-positive clients slated for release from Maryland Correctional Facilities
Engage individuals who are newly diagnosed or have fallen out of care to Primary HIV health care
Provide access and linkage to HIV health care for client.
Ensure documentation in the medical record and HRSA CAREWare database currently used to track MAI outreach services activities and outcomes
Accompanying patients to medical appointments
Verify patient HIV primary care appointments
Provide counseling and support regarding the importance of HIV treatment and adherence for improved health outcomes, reduction in HIV transmissibility to partners,
HIV rapid testing, counseling and linkage to care, referral to PrEP services and/or partner notification services.
Referrals/linkage for pre-conception counseling services for both HIV infected or sero-discordant couples, HIV Obstetrical services, drug treatment and other referrals as indicated.
Provide patient access to research opportunities.
Attendance to at least one HIV related training relevant to this position. May have opportunities for more training
Participation in peer support groups in the community. Facilitate discussions regarding safer sex strategies, sexually transmitted infections and accessing care.
Act as a patient advocate for access to research and clinical protocols.
Possible opportunities for public speaking to professional groups and political interest groups as a representative of the Johns Hopkins Women’s HIV program, at a local and national level.
Bachelor’s degree in related field; some related work experience in the medical field and/or with HIV.
Additional education may substitute for required experience and additional related experience may substitute for required education, to the extent permitted by the JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Additional related experience may substitute for the required education on the same basis. For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job. *

Experience and/or knowledge level to function independently.

Experience and/or knowledge level compatible with functioning as an advisor on issues specific to HIV on a community (CAB’s), local and national level.
Must have proven HIV counseling or social work experience for minimum of 2 years.
