Responsible to the COHE Program Manager, the Workers' Compensation HSC works within/among Harborview (HMC) and UW Medical Center (UWMC) outpatient and inpatient clinical practice areas to facilitate usage of occupational health best practices (OHBP) among health care providers, including occupational and environmental medicine, primary care surgical care, and specialty care. Particular goals include preventing administrative delays that contribute to patient disability through: care coordination/claim tracking/barriers assessment for workers who have had an injury on-the-job or an occupational disease as well as interaction between various workers' compensation stakeholders, including the Washington Department of Labor and Industries (L&I), Self-Insured Employers, health care providers and clinic staff, vocational counselors, and employers. The HSC will participate in OHBP outreach, education/training, quality/process improvement efforts, and back-up Program Assistant work. HSC support a team of 12 HSCs with direct and/or indirect workers' compensation questions and/or patient EHR/workers' compensation research. HSC manages a caseload that supports HMC/UWMC ED and several low-volume workers' compensation clinical areas with patient needs/recommendations.Under general direction, the Workers' Compensation HSC performs work using knowledge and experience regarding the care needs of injured worker patients, providers and staff; contractual agreements with L&I; and hospital policies, procedures and resources. Patient population includes catastrophic claims/injuries and/or complex claims, with a mission-based focus (e.g. low income, non-English speaking, uninsured/underinsured, etc.). Exercise independent and joint judgment in interpreting and applying rules and regulations. Extensive involvement face-to-face and telephonically with health care providers, patients, health service coordinators, clinical staff, and/or the public regarding specific workers' compensation claims, OHBP, policies, procedures; maintain/review case management data, draft claim related notes, memos and correspondences; and participate in planning/implementation/monitoring of continuous improvement projects.MAJOR RESPONSIBILITIES:Health Services Coordinator Lead 25-35%:
In-person and telephonic support to outpatient HSCs with workers' compensation questions.
Review patient EHR and claim information to assess needs and recommendations.
Promote OHBP by mentoring and/or training/co-training HSC team members and other Harborview staff.
Confer regularly with L&I representatives, employers and agencies regarding the interpretation and implementation of program and institutional policies.Health Services Coordinator 65-75%: Identify and Track Claims for HSC Services
Screen workers' compensation claims and identify claims that will benefit from health services coordination; uses internal EHR systems and L&I databases.
Track claims receiving HSC work and identify claims that need further HSC services.
Access the L&I claim file through L&I's Claim and Account Center (CAC) or centralized COHE information tracking or management system.In-person or Telephonic Patient Care Coordination
Act as a liaison, on behalf of health care providers participating in the COHE program (APP), between the injured worker, employer, L&I claim manager, vocational counselor, etc; follow-up or assistance on specific claims. Ensure paperwork/documents are forwarded and received by appropriate party.
Help internal and external L&I stakeholders navigate the UW Medicine and workers' compensation system.
Coordinate care needs for patient, including access to primary and specialty care; provide support to the patient/family by locating appropriate staff to address concerns.
Answers COHE line and responds to patient, employer and referring provider questions in a professional and timely manner.
Maintain contact list of key clinical partners, including roster of participating providers.
Interface with Health Information Management and Release of Information to ensure records are accessed and released in accordance with HMC privacy policies.
Facilitate Stay at Work/Return to Work processes through communication with employer, patient and other stakeholders.
Develop optimal procedures for tracking and proactively addressing communication and care coordination needs for injured workers.
Track specific claims in order to ensure early return-to-work services, care coordination, and improved clinical outcomes of injured workers, as appropriate.
Write accurate case notes which document services provided. HSC case notes must include the content required by the L&I HSC case note format. The content must include specific fields and use a designated formatting within our electronic medical record and billing system.
Provide oversight and problem-solving for catastrophic, complex and standard claim management issues.
Review and respond to COHE general emails within 24 hours or defer to another team member for support/follow-up.
May perform State Fund back-up HSC work for COHE clinics related to coverage or when new staff is being trained.Quality Improvement/Process Improvement
Foster a culture of continuous improvement (PI/QI), providing quality assurance guidance related to OHBP and workers' compensation for participating COHE clinics, including health care providers, HSCs and staff.
Provide QI/PI training, mentoring and support to clinic HSCs. Collect and discuss areas for improvement with COHE Manager/team members.
Coordinate or co-coordinate QI/PI projects including intranet, internet, learning modules, etc.
Attend meetings and/or conferences as program representative.
Use word processing, graphics, statistical, spreadsheet, PowerPoint and/or database software to capture, monitor/review or update data daily, weekly, monthly, quarterly or yearly.
Create or co-create print media for use with outreach and educational elements of COHE program objectives.
